Admission Process
Admitting a patient to the hospice program is a simple process for the attending physician, discharge planner or social worker. Just call the hospice and a nurse will visit the patient and family at home or in the hospital, where information about the program will be presented. If the patient elects the program, the hospice team makes all necessary arrangements for admission.

HOSPICE MEDICAL director
Provides the primary link and involvement
at every stage of the ongoing care of the patient.
REGISTERED NURSE/LICENSE
VOCATIONAL NURSE
Skilled nurses who are trained specifically to deal with pain and symptom management, available 24 hours a day.
SOCIAL WORKER
Counseling/bereavement services and community referrals.
CERTIFIED HOME HEALTH AIDES & HOMEMAKERS
To assist with personal needs.
VOLUNTEERS
To provide companionship and assistance through visits, writing letters, legacy building, reading and or shopping. Bereavement Support for your family and love ones throughout your illness and for up to 12 months following end of life.
CHAPLAINS
Available to provide spiritual support if needed to patient families and caregivers.
